Workforce Development

CHW Training Academy

Sixteen-week intensive certification program combining classroom instruction in chronic disease management, motivational interviewing, and social determinants navigation with 240 hours of supervised clinical practicum in partner health systems. Graduates receive Maryland DHMH Community Health Worker certification and individualized career placement support.

Health Needs Assessment

Comprehensive mixed-methods community health assessments using door-to-door household surveys, focus groups, key informant interviews, and GIS-mapped health resource inventories. Our assessment methodology has been peer-reviewed and adopted by three other state health departments for their community health improvement planning cycles.

Mobile Clinic Network

Technical assistance and implementation support for launching mobile health clinics in medically underserved areas. Our mobile units provide primary care screenings, chronic disease monitoring, immunizations, and care navigation services in neighborhoods where fixed-site clinics are absent or inaccessible due to transportation barriers.

Health Equity Dashboard

Open-source, real-time data visualization platform aggregating ZIP-code-level health indicators including emergency department utilization, chronic disease prevalence, food access scores, environmental hazard proximity, and social vulnerability indices. Used by three Baltimore-area health departments for resource allocation decisions.

Pipeline

Youth Public Health Pipeline

Summer immersion program for Baltimore City public high school juniors and seniors exploring careers in public health, epidemiology, and community medicine. Participants complete a mentored community health research project, shadow CHWs in the field, and receive college application support for health sciences programs at partnering universities.
